We are aware that medical appointments can be particularly triggering for survivors so we have created a document which you can bring to medical appointments and will help the medical practitioner know best how to support you.
The document has spaces where you can adapt/add/edit what is already there so that it is reflective of your needs.
It might be worth keeping hold of a copy of the document so that you can bring it to further appointments in case the medical practitioner doesn’t remember the contents.
We have two versions – one has the Survivors’ Network information on it, the other does not if you do not want to disclose your survivor status in your appointment.
